We'd like to hear about your experiences of palliative and end-of-life care. This includes people who’ve experienced this within the last two years in their family, as carers of family members or friends, or those currently experiencing end-of-life care.

If you live in South Gloucestershire and are over 18, we would like to talk to you. 

We want to find out what matters most to you and your family about end-of-life care, what support you receive in your community and what support you’d like to receive, and to identify what the gaps are in support and care. 

End of life care is defined by NHS England as care that is provided in the 'last year of life'.
