GP surgeries and Primary Care Networks

Primary care networks (PCNs) are made up of multiple GP surgeries that work together in a specific area. The groups of surgeries will also work with local community, mental health, social care, pharmacy, hospital, and voluntary services.

PCNs usually cover between 30,000 and 50,000 patients.

One Care have created a tube-style map which shows which PCN your GP surgery is part of. The name of the PCN is the text in bold at the end of each tube line. For example, Gloucester Road Medical Centre and Horfield Health Centre are part of Phoenix PCN.

Locality Parterships

Each PCN falls under a Locality Partnership, and these are also shown on the map. LPs are groups of NHS providers that work together to deliver care. In the Bristol, North Somerset, and South Gloucestershire area, there are six LPs:

  • Woodspring (North Somerset)
  • Weston, Worle & Villages (North Somerset)
  • North and West Bristol (Bristol)
  • Inner City and East Bristol (Bristol)
  • South Bristol (Bristol)
  • South Gloucestershire (South Gloucestershire).

Integrated Care Systems

Integrated care systems (ICSs) oversee health and wellbeing services. There is an ICS in every part of England - the ICS in our local area covers Bristol, North Somerset, and South Gloucestershire (BNSSG). The King's Fund have created a useful video, explaining this new way the NHS is structured.
